When researchers finally went looking for the relationship between step counts and health outcomes, they did not find a magic threshold. They found a steady curve, climbing fastest at the beginning, and flattening out earlier than the slogan suggested.
The first major dataset came from Lee's team at Harvard. Their 2019 study, published in JAMA Internal Medicine and hosted online by Stanford, followed 16,741 American women with an average age of 72 and tracked their step counts using accelerometers. Women who averaged about 4,400 steps a day had a 41% lower risk of dying during the follow-up period than those who averaged about 2,700. Mortality rates kept dropping as step counts rose — but the curve flattened out around 7,500 steps a day. Beyond that, walking more did not appear to add much in the way of extra protection for this group.
A larger international meta-analysis followed in 2022, pulling together data from 15 cohort studies for The Lancet Public Health. Adults in the most active step quartile had a 40% to 53% lower risk of all-cause mortality compared with the least active. Crucially, the meta-analysis showed that the plateau depends on age: for adults aged 60 and over, the mortality benefit leveled off somewhere between 6,000 and 8,000 steps a day. For adults under 60, the curve kept rising a little longer, flattening out closer to 8,000 to 10,000.
The Lower End Matters More Than You Think
A 2023 meta-analysis in the European Journal of Preventive Cardiology — covering 17 cohort studies and more than 226,000 participants — pushed the floor even lower. The researchers found that as few as roughly 4,000 steps a day were associated with significantly reduced all-cause mortality risk, and that each additional 1,000 steps was tied to about a 15% relative reduction in that risk. Each extra 500 steps was associated with about a 7% reduction in cardiovascular mortality. The takeaway is simple and a little freeing: most of the relative gain comes from leaving "very low activity" behind, not from chasing a high number.
The 2025 Synthesis: Why 7,000 Is the New Anchor
The strongest current evidence comes from a 2025 systematic review and dose-response meta-analysis led by Professor Melody Ding at the University of Sydney, pooling 57 studies across 34 cohorts, summarized by the American College of Cardiology. Compared with a low baseline of 2,000 steps a day, walking about 7,000 steps a day was associated with a 47% lower risk of all-cause mortality, a 25% lower risk of developing cardiovascular disease, a 47% lower risk of cardiovascular death, a 14% lower risk of type 2 diabetes, a 38% lower risk of dementia, a 22% reduction in depressive symptoms, a 28% lower risk of falls, and a 6% lower risk of some cancers. For most of these outcomes, the curve flattened at or near 7,000.
In Ding's words, "aiming for 7,000 steps is a realistic goal based on our findings, which considered evidence across multiple health outcomes." It is, by some distance, the most carefully assembled answer we have to the question of how many steps the average adult actually benefits from.
